2021 W2 AND FORM 1095-C INFORMATION

All W2s for 2021 are located in Workday.

If you are a former employee as of 1/1/22, your 2021 W2 will be postmarked by January 31, 2022, and mailed to your home address on file as of January 31, 2022.

W-2 Reprint and Re-Issue Information

I did not receive my 2021 W-2. How can I get a copy?

 

  • You are able to have your previous manager reset your log in credentials for Workday so you can log in and obtain from the website.
  • Access the Request for Duplicate or Corrected Tax Form document. You will need to complete, sign the document in the presence of a notary and mail the document as directed to request a duplicate copy of your 2011-2021 W2. Please allow 1-2 weeks for processing.

Do you need to report a discrepancy with information as reported on your W2?

Access the Request for Duplicate or Corrected Tax Form document. You will need to complete, sign in the presence of a notary, and mail this document, including any supporting documentation, as directed to request review for a correction of your 2011-2021 W2. Please allow 1-2 weeks for processing.

Need a copy of your W2 from tax year 2010 and prior?

If you need a duplicate copy of form W2 from tax year 2010 or prior, you may order copies directly from the IRS using form 4506T (Request for Tax Transcript).

1095-C Reprint and Reissue Information

What is a Form 1095-C?

The 1095-C contains detailed information about the health care coverage provided by your employer. If you were enrolled in health care coverage, worked an average of 30 or more hours per week or were considered a full-time employee at any point in 2021, then you should receive a 1095-C. You will need it to complete your 2021 tax return. The information you report from the form will be used to determine whether you will pay a penalty for failing to have health care coverage as required by the Patient Protection and Affordable Care Act, also known as "Obamacare."
