2020 W2 AND FORM 1095-C INFORMATION

If you are a former employee as of 1/1/21, your 2020 W2 will be postmarked by January 31, 2021, and mailed to your home address on file as of January 1, 2021.

W-2 Reprint and Re-Issue Information

I did not receive my 2020 W-2. How can I get a copy?

Enter the 5-digit Employer Code: 12939

  • Enter your User ID which is your social security number (SSN)
  • Enter your PIN:
    • If you have never logged into the system, you will need to enter your Default PIN which will be the month and day of your birth followed by the last 4 digits of your social security number (MMDD####). You will then be prompted to enroll in the Authentication program. This step is designed to ensure secure access to your information.
    • If you have previously enrolled and logged into the system but have forgotten your PIN, please click on the "Forgot my PIN" link and follow instructions on how to obtain your current PIN.

    Access the Request for Duplicate or Corrected Tax Form document. You will need to complete, sign the document in the presence of a notary and mail the document as directed to request a duplicate copy of your 2011-2020 W2. Please allow 1-2 weeks for processing.

    While we understand that the timing of this process may not be aligned with your current expectations, we sincerely hope that you understand that this process is in place to ensure that your privacy is protected due to the sensitive nature of these important documents. You may designate the desired delivery option (Email, Fax, or U.S. Mail) which may reduce the timeline to receive your documents.


    Do you need to report a discrepancy with information as reported on your W2?

    Access the Request for Duplicate or Corrected Tax Form document. You will need to complete, sign in the presence of a notary, and mail this document, including any supporting documentation, as directed to request review for a correction of your 2011-2020 W2. Please allow 1-2 weeks for processing.

    Need a copy of your W2 from tax year 2010 and prior?

    If you need a duplicate copy of form W2 from tax year 2010 or prior, you may order copies directly from the IRS using form 4506T (Request for Tax Transcript).

    1095-C Reprint and Reissue Information

    What is a Form 1095-C?

    The 1095-C contains detailed information about the health care coverage provided by your employer. If you were enrolled in health care coverage, worked an average of 30 or more hours per week or were considered a full-time employee at any point in 2020, then you should receive a 1095-C. You will need it to complete your 2020 tax return. The information you report from the form will be used to determine whether you will pay a penalty for failing to have health care coverage as required by the Patient Protection and Affordable Care Act, also known as "Obamacare."

    Can I get my P.F. Chang's/Pei Wei Form 1095-C Electronically?
